We have done our best to ensure washroom access and easy trail options, with spots to rest, but please ensure that you are well enough to walk. Advance Care Planning and Substitute Decision Makers

Friday, October 13, 2023 from 1PM – 3PM

Who will make healthcare decisions for you when you are unable and are they equipped to honour your wishes?

•             Learn who is your substitute decision maker (everyone in Ontario has one) and if you are someone’s substitute decision maker

•             Learn how to ensure they understand your goals, values, and wishes, so you feel they are able to speak for you in the event you are unable

•             Learn what you need to know and understand if you are to speak on the behalf of loved ones as a substitute decision maker

This is important information for everyone, at any age.

The event is open to everyone and will take place on Friday, October 13th at 1 pm. It will be held both virtually on Zoom and also in person at the Evergreen offices in Markham:

Suite 201, 5762 Highway 7 East, Markham, ON, L3P 1A8.

Death Cafe

Join us in a candid and open, virtual, conversation on death and dying. This is open to everyone.

“At a Death Cafe people, often strangers, gather to eat cake, drink tea and discuss death. Our objective is to increase awareness of death with a view to helping people make the most of their (finite) lives’. A Death Café is a group directed discussion of death with no agenda, objectives or themes. It is a discussion group rather than a grief support or counselling session.” – Deathcafe.com

This is open to Everyone = Volunteers, Staff, Clients, and Community.

Please see Death Café’s website if you would like more information on Death Cafés https://deathcafe.com/
