Evergreen Palliative Physicians

The Evergreen Palliative Care Physicians are an independent team of family physicians with a special interest in providing home palliative care. They work closely with Evergreen, HCC case managers, home visiting nurses and clinical nurse consultants to provide care and support in the home.

About Our Physicians

Dr. Andrew Patterson 

Dr. Andrew Patterson is the Palliative Care Lead at Markham Stouffville Hospital. He graduated medicine from McGill University and practiced in Montreal, rural Nova Scotia and West Germany before settling in family practice at Stouffville Medical Centre from 1984 to 2016. Dr. Patterson was the original Chief of Family Medicine at Markham Stouffville Hospital and has also served as Chief of Rehab, Palliative Care and Complex Continuing Care at the hospital. In addition to providing home palliative care, Dr. Patterson works on the inpatient palliative care unit at Markham Stouffville Hospital and is responsible for the training of family medicine residents on their palliative care rotations. Dr. Patterson also teaches palliative care to other healthcare providers and is a certified Pallium LEAP instructor.

Dr. Lydia Cheung

Dr. Lydia Cheung graduated from University of Toronto and was awarded the Eva Weissberger Memorial Award for exceptional clinical skills and patient advocacy in her family medicine residency program. In addition to providing home palliative care, she has a family practice at Thornhill Medical Centre and also provides outpatient palliative care services at the Pain and Symptom Management Clinic at Markham Stouffville Hospital.

Dr. Crystal Lai

Dr. Crystal Lai was part of the first graduating class of family physicians from the Markham Family Medicine Teaching Unit affiliated with the University of Toronto. She went on to complete a Palliative Care fellowship at the University of Toronto. In addition to home palliative care, Dr. Lai has a family practice with Thornhill Family Physicians and also provides outpatient palliative care services at the Pain and Symptom Management Clinic at Markham Stouffville Hospital.

Dr. Jeanne Huo

Dr. Jeanne Huo graduated from family medicine residency at the University of Toronto. In addition to family practice and providing home palliative care, Dr. Huo works in the Pain and Symptom Management Clinic and provides on-call operating room assist coverage at Markham Stouffville Hospital. Dr. Huo has an interest in teaching and has taught clinical skills workshops for first and second year medical students, winning a University of Toronto MD Program Teaching Award for Excellence in 2019.

Dr. Harold Yuen

Dr. Harold Yuen is a palliative care physician who provides home palliative care in the Markham/Stouffville area and also works in the Pain and Symptom Management Clinic in the Shakir Rehmatullah Cancer Centre at Markham Stouffville Hospital.  He helps see patients and families at the Margaret Bahen Residential Hospice and at Southlake Regional Health Centre in Newmarket as well.  Dr. Yuen graduated from medical school and family medicine at Queen’s University, and completed his palliative care training at the University of Toronto. He is the Medical Lead for Hospice Palliative Care Teams for Central LHIN, a LEAP facilitator, Lecturer at the University of Toronto and Peer Assessor at the CPSO.
